Transition Aged Youth Services
Transition Aged Youth [TAY] services are targeted to individuals ages sixteen to twenty-two with serious and disabling conditions.
For those with mental illness, we provide case management services designed to support the individual so that he or she can complete high school and become gainfully employed. Our case managers provide intensive assistance with:
- Obtaining benefits
- Coordination of treatment
- Medication monitoring
The TAY program can also provide financial support necessary for participation in supplemental educational activities such as:
- Drivers' education
- Music lessons
- Gym memberships
- Tutoring
PDG can also provide vocational rehabilitation and employment services to qualified participants served by the Maryland Division of Rehabilitation Services.
